Forms & Registration Information

Registration

The two seasons of play are Fall and Spring. Fall season games typically start the end of August and go through mid-October. Spring season games start mid-April and go through mid-June.

Items Needed for Registration

1. Registration Form (click link at bottom of page to download)

MUST be filled out completely, and signed on both the waiver of  liability section, consent for medical treatment section, and code of conduct section.

2. Player's birth certificate

(non-returnable photocopy) will be needed for new players.  This should be a photocopy of the official birth certificate (baptismal records and hospital birth records are not valid).  If they have played with Strikers in the past, then we should already have a copy of their birth certificate on file and will not need an additional one.  If you want to check if we already have it, contact the registrar.

3. Two (2) small color photos ARE required for U9 and up.

These will be put onto their player cards, which are required by GAASA.

4. Payment

Current Club Fees are $135 (Make all checks payable to Strikers Soccer Club)

U9-U14 Uniform Fees are $90 - This includes 2 jerseys, 1 pair of shorts and 1 pair of socks

Special note for GAASA league teams:  Forms that are not complete (missing information or missing signatures), missing payment, missing birth certificate, etc. will be sent back with a note listing what is missing and a date by when it must be returned in order to be registered.  Please realize that you are not yet registered if your forms get returned, and we will be unable to officially fill your player's spot on any specific team until the forms are returned correctly and completely.  Also, forms completed after the deadline will incur late charges.
